These charts can help you track your daily medications and antibiotics to ensure you take them correctly and on time. How to Use Your Medication Chart:
Write Down Your Medicine
Find the name of your medication on the bottle and write it in the chart.
Write how much you need to take (for example, 1 pill or 5 mL).
Write how many times a day you need to take it (like 2 times a day).
Set Your Schedule
Pick the times when you will take your medicine.
Try to take it at the same time every day.
Mark Each Dose
Every time you take your medicine, put a checkmark (✓) in the box for that time.
If you miss a dose, write it down and ask your doctor or pharmacist what to do.
Watch for Side Effects
If you feel sick after taking your medicine, tell an adult or call your doctor.
Write down any side effects you notice in the chart. The notes section is also a great place to record blood pressure, blood sugar or other tracking information.
Finish All Your Medicine
Even if you feel better, take all your medicine until it's gone.
Mark off each dose until the chart is full.
Keep Your Chart Safe
Put your chart somewhere you will see it every day, like on the fridge or by your medicine.

Once Daily Medications: This chart is helpful for patients with medications that are given at one time of the day only. This chart can help you track your medications for a week.
Twice Daily Medications: This chart is helpful for patients with medications that are given two times a day. This chart can help you track your medications for a week.
Three Times Daily Medications: This chart is helpful for patients with medications that are given three times a day. This chart can help you track your medications for a week.
Four Times Daily Medications: This chart is helpful for patients with medications that are given four times a day. This chart can help you track your medications for a week.

These charts are designed specifically for antibiotics and are available for the following schedules:
Once Daily Antibiotics: This is for one time daily antibiotic doses and has enough space to chart up to 10 days.
Twice Daily Antibiotics: This is for two times a day antibiotic doses and has enough space to chart up to 10 days.
Three Times Daily Antibiotics: This is for three times a day antibiotic doses and has enough space to chart up to 10 days.
