P4 Rotation with Wright Geriatric Consulting: A Unique Pharmacy Consulting Experience

Welcome to Wright Geriatric Consulting, where we offer P4 students the opportunity to engage in a meaningful and hands-on pharmacy consulting rotation. Our mission is to empower students to develop expertise in age-friendly pharmacy consulting and quality improvement, while making a tangible difference in the lives of geriatric patients.

What to Expect During Your Rotation

Onsite Projects students will participate in:

  • Medication Reviews and Reconciliation: Collaborate with care teams to review medication regimens, ensuring safety, efficacy, and alignment with geriatric care principles.

  • Regulatory Compliance Support: Assist facilities in meeting state and federal pharmacy regulations through audits and documentation.

  • Care Team Integration: Shadow and work alongside pharmacists as they consult with nursing and administrative staff to improve patient outcomes.

Offsite and Quality Improvement Projects to gain experience in the following:

  • Quality Improvement Initiatives: Analyze and propose solutions for recurring challenges, such as optimizing medication use in dementia care or reducing polypharmacy.

  • Policy Development: Create or update policies and procedures tailored to age-friendly principles and compliance requirements.

  • Data Analysis: Contribute to projects involving medication error tracking, outcomes analysis, and benchmarking for best practices in geriatric care.

Virtual/Distance Rotation Practices
When onsite participation is not feasible, virtual or distance rotation practices may be offered, subject to approval by the College of Pharmacy. These activities will align with the rotation's goals and may include:

  • Remote participation in quality improvement initiatives.

  • Virtual medication reviews and consults with care teams.

  • Online research and policy development projects.

Unique Opportunities

  • Age-Friendly Care Integration: Learn to apply the 4Ms framework of Age-Friendly Health Systems: What Matters, Medication, Mentation, and Mobility.

  • Interprofessional Collaboration: Work with diverse care teams, including nurses, social workers, and administrative staff, to support holistic care.

  • Professional Development: Attend relevant conferences, webinars, or training sessions, such as those offered by the American Society of Consultant Pharmacists (ASCP).
